There are more than a few schools in Israel that offer Hebrew studies for international students and visitors so there isn't only one application for - for each one you'll have to fill a different form.
Among those institutes: Ulpan Gordon, Levinsky College, Ulpan Israeli, IDC Herzliya and The Tel Aviv University.
Some of them may offer scholarships or financial aid so i advise you t contact a few of them (at least) to see who can offer you the best course in the nest price.
