In order to start a program for doctors you'll have to contact the different hospitals in Israel directly so they could tell you what is it you need to send them in order to apply.
Among those hospitals you can find The "Soroka" medical center in Beer Sheva, Ziv medical center in Safed, Sheba medical center in Ramat Gan, Rambam hospital in Haifa and more.
You should know that it's usually harder to get in to a program in the hospitals in the center of Israel and in Jerusalem - the hospitals outside of those place are usually in search of great doctors and are more open.
