Extension for unpaid fellowship in Interventional cardiology

I have been in SZMC since Feb 5th of this year for a fellowship in Interventional cardiology ,the approval by MOH is till 30th Nov of this year, so too my visa expires on 30th Nov .
I wish to extend my training program ( unpaid, self supported) for a year more till Nov 2024 , do I need to take any exams , if not , or yes can I get an official clarification for the same , what are the various formalities I need to fulfill to get an extension for one year by MOH besides the letter of recommendation for the same by the chief hospital.
