Fellowship in Opthalmology

I have completed MD in Opthalmology from India and intend to complete a fellowship in glaucoma/squint from centres such as Rambam/Sourasky/Hadassah.

My query is I have heard you need a permit to practice in Israel, especially for surgical fellowships. Does the Ministry of Health issue it? Funding is not an issue for me and I don't want it to be merely observship but also want clinical and surgical experience.
